Let’s Take Action on Patient Goal-Setting
As introduced in the November BC Kidney Days session,
Journeying Together: Creating a Map to What Matters Most
to Patients, people living with kidney disease and their care
teams can now access and use a practical strategy document
and its companion worksheet to improve patient goalsetting. The strategy document includes several tools and
how to use them. We encourage all care teams and patients
to have a look and give the tools a try!
