Being prepared in the event of a disaster is important, especially for dialysis patients. This section has a number of resources to help you prepare.
- Emergency Preparedness Information for Dialysis Patients booklet: For a full description of what to do during an emergency, please download this PDF document. We recommend printing a copy to have available in case a disaster occurs. This package includes forms that you can fill out to record important, personalized information that is useful to have during an emergency (e.g., your doctor’s contact information, details about your medications, etc.).
Below are forms from the Emergency Preparedness booklet that can be individually printed and filled out:
