Supporting Living Donor Kidney Transplants: A New Resource for Social Workers

​Living donor kidney transplants are often the preferred treatment for patients with end-stage renal disease because they offer the best chance for improved quality of life. In recent years, BC Renal has partnered with BC Transplant on the Transplant First initiative, which promotes pre-emptive transplant (living donor transplants before dialysis begins).  

As this initiative has grown, so has the role of home renal teams in guiding patients through the transplant journey. Social workers are central to this process, providing support with donor outreach, financial planning, and addressing psychosocial concerns.  

Recognizing that education on transplant practices varied across sites and health authorities, the BC Renal Social Work Professional Practice Council identified a need for consistent resources. In response, the Council developed the Social Work Transplant Reference Guide—a comprehensive tool that brings together existing education from BC Renal and BC Transplant in one place.  

The guide includes:  

  • Key topics and common patient concerns related to transplant.  
  • Suggested interventions for social workers.  
  • A list of financial and community resources to support patients.  

During development, the team also created three new patient handouts, reviewed by patient partners to ensure clarity and relevance.
